Physician Access Info
San Luis Diagnostic Center is a completely digital facility. This means that once the radiologist interprets the study, the referring physicians can access
patient exams and reports online.
For referring physicians requesting online access to our database, please fill out and print the Physician Consent for Database form below. Fax the
completed and signed form to (805) 542-9060 (ATTN: Webmaster).
Upon receipt, information will be added / updated in the San Luis Diagnostic Center's database, and a login / password will be created for and sent to the referring physician.
Physician Access to the SLDC Database:
For optimal access install both viewers in the following order.
1. MI View (Basic Viewer): https://imaging.sldiagnostic.com/
2. HRS (Advanced Viewer): https://imaging.sldiagnostic.com/hrs
